- Talk to Phil... or else.There are two forms attached to this wiki page: Direct Deposit Form and Business Mileage Worksheet. Fill these forms out.
View file name Direct Deposit Setup.pdf height 250 View file name Buisness mileage reimbursement worksheet (1).pdf height 250 - Write your full name along with your UID on the back of your receipts.
- Return the completed forms and receipts to an Eboard member (you can scan documents and send via email), and you're ready to get reimbursed!
